Calling a Spanish number from a Spanish fixed line carries a surcharge, and it exists because of local regulation.
When it applies
A surcharge is added to calls that go from a Spanish fixed-line number to a Spanish number. Both ends Spanish, with the calling number a fixed line.
Calls to Spain presenting an international number do not attract it, and neither do calls from a Spanish mobile number.
Why
Spanish regulation. The surcharge exists to comply with it, not because that route is more expensive to carry.
What to check
If you have people in Spain calling Spanish customers, this is worth knowing before the first invoice rather than after. The pattern is common enough that it catches companies who set up a Spanish office and present a Spanish landline.
